Goal Statement:

Reduce morbidity and mortality associated with end-stage renal disease and increase patient choice by improving access to alternatives to center-based dialysis. Starting from the baseline for the calendar year ending December 31, 2019, by September 30, 2021.

Increase by 10 percent the number of new end-stage renal disease patients on home dialysis.

Increase by 10 percent the number of kidney transplants performed.

Goal statement’s baseline period of measurement was updated from CY 2018 to CY 2019 to reflect availability of more current data.
